How do dentists remove a tooth rotten to the gum line?

My boyfriend has several teeth that are rotten to the gum line. He is very reluctant to go to a dentist because of a phobia. I am worried that they will not be able to do anything because they wouldn't have anything to 'pull' on. Is his only option surgery?
